hi, i would like to ask regarding my surname. I was born in 1985. My mother registered my name under my father's surname but the problem is my mother and my father is not married. Right now, in all my documents and credentials I am using my father's surname. Is this legal or do I have to change my surname to my mother's surname? Also, can my mother be my dependent in my Philhealth and other insurances even if I'm using my father's surname while my mother is still using her maided name? hope you can helo me on this one.

did your father sign your birth certificate? if not, can you still contact him to issue an affidavit of acknowledgment?

you are required to use the name in your birth certificate.

hw didn't sign in my birth certifcate,although he's the one who picked up my birth certifcate in the municipal hall.my father's is still alive, but i dont have contact on him.does that mean, since he didn't sign my birth certificate, i am not supposed to use his surname? My worry is can i claim my mother as my dependent?

even you are using your father's name, that will not be a problem.

thanks for your fast reply. this was been my worry ever since.. i am afraid of that there will come a time that i can not declare my mother as my dependent because we have different surname.. do i still have to get my father's affidavit of acknowledgment?

if your birth certificate states the name of your mother then you can declare her as your dependent
