Arresto Menor
our child was born out of wedlock and use my surname. after three years, we decided to use his father’s surname. after processing some documents, theres an attachment that sayiNg our child now can use his fathers surname. is it possible to totally ammend or change the previous mothers surname into his fathers surname on the child’s birth certificate?

attachment? or annotation on the margin? if there is an annotation, that is all it takes to change the name. no new one will be issued.

attachment po, there’s no remarks or attachment written on the margin of the childs birth certificate, so everytime na magrequest ng birth certificate nya, laging nakasama ung attachment na acknowledgement ng father nya.

in that case, that is the proof that he can use the father's surname.
