I was employed as an agent but was assigned as a QA when our account went live on Dec 2013. There was no black and white or evaluation form that was given to me for me to either get the job or not.

There was a standard in the company that usually, you cannot get promoted until regularization unless maybe there is an evaluation form.

Last April, I was promised to get the title. When I spent the whole month doing a follow up, my manager just told me that he will not give me the title since I was not able to meet my goals.

I did not sign anything when it comes to goals that needed to be attained.

He then started the evaluation form only after regularization which was wrong in my perception since I was promised the job already, although verbally though. And there was no expectation given that I need to have an evaluation after only regularization.

I asked for HR's help - only to be told that I will get a feedback and nothing was given to me up to now.

I already resigned immediately because I felt insulted.

Can I file a case in NLRC for this one?

You can, but I doubt you will win.
