Birth Certificate wrong declaration of parents marital status

Hi po gusto ko sanang mgpatulong regarding s birth certificate ng anak ko, ang nkalagay s BC nya married kmi ng father nya pero ang totoo hind po inilagay ko lng yun noon s ospital noong ipinanganak ko sya para magamit nya ang surname ng father nya, kaya mg mula ng pumasok sya s school surname ng father nya ang ginamit nya ngayn po college n sya, pg mg apply na po sya ng trabaho specially abroad ang unang kinukuha birth certificate nya at pg nkitang married ang parents ang ksunod n kkunin marriage certificate ng magulang, hind nman po kmi kasal kaya wala syang m provide pano po kaya yun, gusto ko p sanang ipa cancel nlang s birth certificate nya yung married n nkalagay doon pero sana po yung surname pa rin ng father nya ang gagamitin nya possible po ba yun?Maraming salamat po.

Wrong info on public document is falsification and punishable by law.
Sapat na sana ang authority to use surname noon.
Nasaan na si Father?
Baka puede kang bumisita sa munisipyo/ cuidad ng office of civil reigistry kung saan mo siya niregister at magtanong sa help desk nila.
Huwag ka ng magkamaling kumuha ng fake na marriage certificate.

Yes i know. That i falsified the documents.  Because as a mother, ayaw ko namang lumaki ang anak ko na walang middle initial.  

Yes wala naman akong balak kumuha ng fake marriage certificate.  
And the yr my daugther was born was 1995. It was the time Cory forbid all illegitimate son and daugther use their father's surname.

Father is already married.

meron bang acknowledgement c father?
nakapirma b sya sa BC ni anak nyo?
san na sya now? pwede p b syang mag provide?

yun lang ang pwedeng way para magamit nya surname ni father since yun ang gusto nyo..

Pwede rin na gamitin nya na lang surname nyo, pero me process din to, medyo mas madali na now if nasa edad na anak nyo..

Yes po ang father willing pumirma, pra i acknowledge ang anak nya at pra magamit ang surname nya, pero nung time n pinanganak ko sya wala syang pirma s birth certificate, pero nung malaman nya n pwed n po ngyn, sya n po mismo ang nagsabi s amin n asikasuhin bka daw po kc pg nag apply ng trabaho bka dun po mgkaproblema, salamat po.

yon naman pala eh,,
punta kayo sa PAO, or any legal office.
pagawa ng affidavit of acknowledgement ng father,
notarized.
then submit sa local civil registrar, cla magsubmit nito sa NSO para sa annotation,
after few weeks.
nakalagay sa BC nya yong surname mo pa rin pero sa right side(annotation) yong acknowledgement ni father, then yong name nya bearing yong surname ni father.
so legal na paggamit nya ng surname ni father at last.
confirm nyo ke registrar kung paano mabago yong married na word sa orig BC, since hindi naman binabago ng NSO ang orig upon annotation.
