I think this is an estimate of how you intend or will be able to pay your obligation put in the simplest term and on paper. It's like a promissory note. It should be made available on your court date (ten days) since in effect that is your appointment with them and no other date/s were given in regard to the pro-forma?
So you gotta have it on Monday 14 Nov.
My opinion: go to the claims court early Monday and see if there are forms to be filled-out. You can go to a attorney-at-law located nearby in case you have to.

Ang sinabi ko po hindi ako pwede magcommit ng hindi ko pa nakakausap ung tao sa paligid ko like si Mama na magpapautang sakin dahil nga wala akong work (Hindi ako pwedeng magYes, ng wala naman akong hawak or ineexpect na pera kasi niloloko ko siya nun). At dahil don, sabi nia na gumawa na lang ako ng letter with three specimen signature, ang sabi ko iemail ko agad. Nung binalik ko na yung cellphone sa messenger nila, naghintay siya sa gate iniintay daw yung letter. Sabi ko iemail ko, kahit agad agad, but then ininsist niya nia kailangan daw ng Handwritten Letter. Binigay niya sakin ulet ang cellphone at sinisigawan ako ng agent na napasinungaling ko daw, manloloko, at may sakit sa pag-iisip. Firm ko sinabi na iplay ang recorded line at pakinggan niya na sinabi ko na mageemail ako asap. Pupunta daw sa Barangay yung staff niya kapag hindi ako nag-handwritten letter, sinabi ko na tatawag muna ako sa bank to talk to them na sakanila ako mag-eemail at nakaCC lang ang CA, nung nasa phone ako (siguro nakita nung messenger na totoong tumatawag ako sa landline) nag-iba yung tono nung agent at hindi na ako sinisigawan, email na lang daw niya sakin ang letter at reply ako within the day. I was meaning to call talaga yung BSP Consumer Hotline wala lang sumagot sa pang3rd na ring, to ask kung gagawa ba ako ng Promisory Letter with 3 Specimen Signature and 2 Govt IDs. Nag-play ko po ang CCTV naming, at talagang nasabi ko nung una pa lang na "cge, email na lang ako."
Kung bumalik po ba siya dito at nanghihingi ng Hand written letter with 3 Speciment siganture, legal po yun at okay lang ba na magbigay ako kahit na sinabi ko na hindi ako pwede magcommit? At kung pwede naman po, Koconfirm ko po yung advice ng friend ko na law student kung sa Barangay naming gawin hindi din po dapat nakaaddress yung letter sa isang company dahil for individual lang ang inaaddress ng mga Barangay, so sa messenger or sa agent dapat iaddress?
