Is "but, my name and signature on my son's BC is indicated" supported with the Authority to Use Father's Surname?
Is the BC an authenticated copy of NSO?
What year was your son born?

Check with the Local Office of Civil Registry where your son was registered on what can be done.

You already have an Affidavit of Acknowledge if you said your name and signature are already on the BC. I wonder why your last name was not picked up as the child's surname. Could it be just a clerical error? Check with the LOCR. Based on the year of date, he is covered by the illegitimate's right to use the father's surname. Let us know what you will discover.
