He died a month ago, and we are processing the death claims in SSS. Problem is, one of the requirements is an NSO-authenticated birth certificate. Sure the name and the birth date he used in SSS are not matching with the information in his birth certificate. Is it possible that I can just make a request from a lawyer to provide me a document certifying that the two names belong to a single person? Can the birth date be also included on that certification?
