Good day po
Tanong ko lang po kung ano ang dapat kong gawin sa birth cert. ng
tatay ko at na auntie ko kasi po yung birth cert ng papa ko (DESEASED)
ang maiden name ng lola ko ay FIDELA MOSENDE DELA COSTA may middle
na MOSENDE. Tapos ang birth cert. ng antie ko ang maiden name ng
lola ay FIDELA DELA COSTA walang middle name na MOSENDE. Gagamitin
ko po yung birth cert nila to prove na pamangkin ako ng antei ko na
nasa Australia for visa appltication sa Australia. Salamat po.

are their mothers on the birth certificate the same? are your lolo and lola married?

Opo iisa lang at kasal po sila. Ano ba ang pwede kong gamiting
documents para patuyang iisa lang ang taong nasa birth certificate nila w/c is my lola. Salamat po

your aunt should ask the civil registrar to correct the birth certificate based on ra 9048. argue that the failure to indicate her mother's maiden name is a clerical error because the mother's name is indicated on the birth certificate.

Pwede po ba yung Mosende na lang po ang patangal para magparehas ang name ng lola ko sa birth certificate nila? Wala po kasi dito ang antie ko nasa Australia po e at sya na lang ang natitirang anak ng lola ko.

then that will take a court order. you will have to file a petition and get a lawyer to assist you.
